Survivorship and Support

St. Luke’s nurse navigators help patients with cancer gain access to essential resources, decreasing barriers to care and establishing an easy route through a cancer journey from diagnosis to treatment and beyond to survivorship. Nurse navigators can help:

  • decrease barriers for patients, whether internal (health system barriers) or external (community or personal barriers);
  • educate patients on their diagnosis, answer questions and provide support throughout the cancer journey;
  • assist with scheduling appointments, procedures and more in a timely manner;
  • partner with the health care team to improve communication and collaboration; and
  • connect to other needed cancer support services to help with financial, transportation or psychosocial barriers that may delay or limit access to care.
